Co-pay:  $25/visit PPO, $35/visit Non PPO

Plan pays 90% in network, 80% out of network

 

Prescription Card  (Merck-Medco)

First of the month following 30 days of employment

Retail:
Generic = $10/fill

Preferred Brand = $30/fill

Specialty/Non-Preferred Brand $50/fill
Mail in:
1.5x Retail for 90 day supply

Vision Care (VSP)

First of the month following 30 days of employment

Single:  $1.05/pay

Emp/Child : $1.77/pay

Emp/Spouse: $1.81/pay

Dental Care (Self-Insured)
